Patient Information & FAQs
New Patients
Prior to a new patient examination you will be sent an email containing information about your appointment and a medical history form to complete
If you are unable to complete the form before your appointment, copies will be available at the practice but, we ask that you arrive a few minutes early to complete this before your appointment time
If you are taking any medications, please bring with you an up to date prescription so that we have accurate details of your current medications and their doses
If you have been seen recently by another dental practice and have had x-rays taken, please bring these with you, or ask your previous practice to send them to office@gwynnedental.co.uk
New patient appointments consist of a full examination and treatment planning for ongoing treatment, unless otherwise arranged with the reception team
When booking a new patient examination, we will take a £45 deposit to secure the appointment, this is taken as a retainer and will come off of the price of your initial appointment once completed
Cancelling or re-arranging appointments
We understand that sometimes appointments have to be re-arranged due to unforeseen circumstances. If you would like to re-arrange an appointment, please contact the reception team on 01256 321945 and they will be able to arrange a new appointment at your earliest convenience.
We request that you give us as much notice as possible to change appointments, with a minimum notice period of 24 hours. Any changes or cancellations of appointments with less than 24 hours notice may incur a charge. This also applies to missed appointments.
